How Hospice works

When a patient elects to receive hospice care, the patient’s doctor and the hospice will work with him/her and their family members to prepare a plan of care. As a hospice patient, care will be provided by a team that can include:

  • Family members
  • Doctor
  • Nurse
  • Counselors, including clergy
  • Social Worker
  • Home Health Aides
  • Trained volunteers

Volunteers are trained to better understand what patients and their families are facing so they are sensitive to emotions and concerns that may be involved. They help with every day tasks, such as shopping, transportation, household chores, respite for caregivers, and simple companionship.

Presbyterian Hospice also provides speech-language, physical, and occupational therapists, as well as other professionals to provide care as needed. These services are covered under the Medicare Hospice Benefit when needed to provide for the patient’s comfort and quality of life related to the terminal illness.

Other aspects of care are also covered under the Medicare Hospice Benefit including prescription medications, medical supplies, and bereavement support. Care and services are provided in the patient’s home, whether that is a retirement community residence, home in the community-at-large, or an adult care home.

Presbyterian Hospice staff are available 24 hours a day, giving patients unlimited access to the support and professional services they need. In addition, choosing Presbyterian Hospice in the Farmington area for those who are in the greater community, means that should you need to move to a care home, the transition can be easier because we have the experience from more than 45 years in the community providing health care services to our residents.
